Job summary

OPCO Skilled Management is seeking a skilled MDS Coordinator to coordinate timely completion of the RAI process and care management from admission to discharge, working under the Director of Nursing and VP of Clinical Reimbursement.

You will ensure compliance with Federal and State guidelines, collaborate with the interdisciplinary team, and maintain accurate MDS coding and documentation across resident records.

Qualifications

• RN/LVN license required.
• Minimum 2 years nursing experience in a Skilled Nursing Facility preferred.
• Strong knowledge of Case-Mix, Medicare PPS, and Medicaid reimbursement.
• Thorough understanding of the Quality Indicator process.
• Knowledge of OBRA regulations and the Minimum Data Set (MDS).
• Knowledge of the care planning process.
• MDS 3.0 experience preferred.

Responsibilities

• Collaborate with Interdisciplinary Team to assess resident needs and schedule MDS assessments and care plan reviews.
• Decide on completing or not completing additional MDS assessments based on clinical criteria.
• Coordinate daily stand up meetings; review resident care and assessment reference dates.
• Ensure compliance with federal and state regulations in MDS coordination.
• Monitor MDS and care plan documentation in the medical record to support coding.
• Maintain current MDS status per state and federal guidelines.
• Enter resident data into MDS programs accurately and timely.
• Complete accurate coding of the MDS using medical records, observations, and interviews.
• Perform other duties as assigned.
