OPCO Skilled Management in Rio Rancho, NM is seeking an MDS Coordinator to support the Director of Nursing and RN Assessment Coordinator by ensuring that documentation meets Federal, State, and Certification guidelines and that the RAI process is timely and complete.

The role requires a RN or LVN license in NM, at least 2 years of nursing experience in a Skilled Nursing Facility, and strong knowledge of Case-Mix, Medicare PPS, Medicaid reimbursement, OBRA and the MDS process to support accurate

Qualifications

• Must hold an active RN or LVN license in NM.
• Minimum of 2 years in nursing within a Skilled Nursing Facility.
• Strong knowledge of Case-Mix, Medicare PPS, Medicaid reimbursement, OBRA and MDS.

Responsabilités

• Assists the Director of Nursing and RN Assessment Coordinator with ensuring documentation meets Federal, State and Certification guidelines.
• Coordinates the RAI process to assure timeliness and completeness of MDS, CAAs, and Interdisciplinary Care Plan.
• Participates in regulatory or certification survey process and helps correct deficiencies.
• Reports trends from audits to the Quality Assurance Committee.
• Monitors RAI process tracking, discharge/reentry and Medicaid tracking forms in PointClickCare.
• Ensures accurate determination of Assessment Reference Date reflecting patient care needs for Medicare/Medicaid payments.
• Submits MDSs to the State timely and maintains validation records of submissions.
