Job summary

OPCO Skilled Management is seeking an experienced MDS Coordinator to oversee timely completion of RAI and care management processes from admission to discharge across New Mexico facilities. You will collaborate with the nursing leadership and interdisciplinary teams to ensure compliance with federal, state, and certification guidelines.

The role requires an RN, knowledge of MDS 3.0, and familiarity with Case-Mix, PPS, and Medicaid reimbursement.

Qualifications

• Graduate of an approved Registered Nurse program and licensed in the state of practice, required.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.
• Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement, as required.
• Thorough understanding of the Quality Indicator process. Knowledge of the OBRA regulations and Minimum Data Set.
• Knowledge of the care planning process.
• Experience with MDS 3.0, preferred.

Responsibilities

• Works with Interdisciplinary Team to assess resident needs and schedules MDS assessments and care plan reviews.
• Ensures team decisions on MDS, assessments based on clinical criteria per RAI User's Manual.
• Coordinate daily stand-up meetings and review resident care and assessment dates.
• Comply with federal and state regulations for MDS process.
• Monitor MDS and care plan documentation in medical records to support coding.
• Maintain current MDS status per state and federal guidelines.
• Enter resident data into MDS programs accurately and timely.
• Code MDS information from medical records, observations, and interviews.
