Taos Healthcare is seeking an MDS Coordinator to oversee timely and accurate completion of the MDS process from admission to discharge, ensuring compliance with federal, state, and certification guidelines. You will coordinate with the nursing leadership and care teams to manage data and documentation across RAI-related activities.

The role requires RN/LVN licensure, SNF nursing experience, and strong knowledge of MDS, OBRA, and care planning.

Qualifications

• Graduate of an approved RN/LVN program and licensed in the state of practice.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility.
• Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement.
• Thorough understanding of the OBRA regulations and Minimum Data Set.
• Knowledge of the care planning process.
• Experience with MDS 3.0, preferred.

Responsabilités

• Collaborate with the Interdisciplinary Team to assess resident needs and schedule MDS assessments and care plan reviews.
• Decide with the team on completing or not completing additional MDS assessments based on clinical criteria.
• Coordinate daily stand-up meetings to review resident care and set assessment reference dates.
• Comply with federal and state regulations regarding MDS process completion and coordination.
• Monitor MDS and care plan documentation to ensure accuracy in medical records.
• Maintain current MDS status per state and federal guidelines and update computerized MDS systems.
• Accurately code the MDS via record review, observation, and interviews with staff, residents, and families.
• Perform other duties as assigned.
