Job summary

OPCO Skilled Management is seeking an experienced MDS Coordinator to ensure timely and accurate completion of the RAI process and care management from admission to discharge in New Mexico facilities. The role requires RN licensure, familiarity with MDS 3.0, Medicare/Medicaid knowledge, and collaboration with the interdisciplinary team to maintain compliant care plans.

Qualifications

• RN licensed in the state of practice, required.
• Minimum 2 years nursing experience in a Skilled Nursing Facility preferred.
• Knowledge of Case-Mix, Medicare PPS, and Medicaid reimbursement.
• Understanding of OBRA and Minimum Data Set (MDS).
• Experience with MDS 3.0 preferred.

Responsibilities

• Coordinate MDS assessments and care plan reviews with the interdisciplinary team.
• Decide on MDS completion based on clinical criteria and RAI manual guidance.
• Schedule and lead care planning conferences; communicate outcomes to staff and families.
• Ensure compliance with federal and state regulations in RAI processes.
• Maintain accurate MDS status and data entry in computerized systems.
• Prepare and submit required MDS documentation to state/federal databases.
• Train staff on RAI processes, PPS, and clinical computer systems.
• Maintain confidentiality and adapt to policy changes.
