Job summary

OPCO Skilled Management is seeking an MDS Coordinator to oversee timely and accurate completion of the RAI process and care management from admission to discharge, in line with policy and Federal, State and Certification guidelines.

Under the direction of the Director of Nursing and VP of Clinical Reimbursement, you will coordinate information systems operations and education for the clinical department, maintaining MDS status and ensuring compliant documentation across state databases and care

Qualifications

• RN licensed in the state of practice is required.
• Minimum of 2 years nursing experience in a Skilled Nursing Facility preferred.
• Strong knowledge of Case-Mix, Federal Medicare PPS, and Medicaid reimbursement; OBRA and MDS 3.0 familiarity preferred.

Responsibilities

• Coordinate MDS assessments and care plan reviews with Interdisciplinary Team.
• Ensure documentation supports MDS coding and complies with regulations.
• Maintain current MDS status according to state and federal guidelines.
• Enter resident data into MDS programs accurately and timely.
• Attend interdisciplinary meetings and communicate changes to staff and families.
