Job summary

SNF Staffing Solutions in Texas seeks an experienced MDS Coordinator to ensure timely and accurate completion of the RAI and care management processes from admission to discharge, in line with company policy and Federal/State guidelines.

You will collaborate with the Interdisciplinary Team, coordinate MDS assessments, maintain records, ensure coding accuracy, and support DON and clinical leadership to optimize resident care and compliance.

Qualifications

• Graduate of an approved RN program with an active license.
• Minimum 2 years of nursing experience in a Skilled Nursing Facility.
• Thorough knowledge of Case-Mix, PPS, and Medicaid reimbursement.
• Familiarity with OBRA regulations and Minimum Data Set processes.
• Strong understanding of the care planning process and MDS workflows.

Responsibilities

• Collaborate with the Interdisciplinary Team to assess resident needs and schedule MDS assessments.
• Decide on completing or deferring additional MDS based on clinical criteria and RAI guidelines.
• Coordinate daily stand-up meetings and set Assessment Reference Dates.
• Ensure compliance with federal and state regulations for the RAI process.
• Monitor and document MDS and care plan status in the medical record.
• Maintain current MDS status and accurate data entry in MDS software.
