This role leads interdisciplinary planning, supports accurate reimbursement, and educates staff on documentation standards within a regulatory healthcare environment. The position requires an RN license, CPR certification, and at least two years of nursing experience, with strong knowledge of RAI, MDS 3.0, and PDPM

Qualifications

• Graduate of an accredited school of nursing.
• Current RN license in the state of practice.
• CPR certification.
• Minimum two years of nursing experience.
• Knowledge of RAI process, MDS 3.0 guidelines, and PDPM reimbursement.
• Experience with interdisciplinary care planning.
• Proficiency with electronic health records (EHR) and MDS software.

Responsibilities

• Oversee completion of all MDS assessments and care plans within required timelines.
• Ensure accuracy and CMS-compliant documentation for MDS data and related records.
• Lead interdisciplinary team through care planning for residents’ physical, psychosocial, and functional needs.
• Monitor documentation quality to ensure accurate coding and reimbursement.
• Coordinate pre-admission screenings and collaborate with admissions for appropriate placement.
• Educate staff on documentation standards and Medicare criteria.
• Participate in QAPI initiatives and interdisciplinary meetings.
• Maintain regulatory compliance and prepare for audits and surveys.
• Follow HIPAA regulations and provide compassionate resident care.
• Perform other duties as assigned.
