Dart Healthcare Staffing seeks an MDS Coordinator to lead the MDS and utilization management processes in accordance with federal and state standards. The role involves developing care plans, coordinating the plan of care, and auditing documentation to ensure regulatory compliance.

The position requires an active RN or LPN license, with experience in long-term care and MDS knowledge. The incumbent will work in a health care facility with multiple responsibilities and strict confidentiality.

Qualificações

• Must hold a current state RN or LPN license.

• Experience with care plan development and discharge coordination.

Responsabilidades

• Stay abreast of federal and state regulations and standards.
• Assist with development of comprehensive care plans with the MDS.
• Coordinate and monitor the written plan of care for each Guest.
• Assist nursing management with discharge coordination and documentation.
• Perform administrative tasks and reporting.
• Ensure infection control and safety practices are followed.
