Job summary

Cambridge Healthcare Services, LLC is seeking an MDS Coordinator to coordinate the resident assessment process. You will develop MDS/CAA and care plans, work with an interdisciplinary team, and ensure compliance with RAI guidelines and facility policies.

You will document accurately, support leadership like the DON/ADON, and participate in surveys, QAPI, and quality improvement initiatives. This role requires nursing licensure and LTAC experience in a coordinated setting.

Qualifications

• RN Nursing Degree or LVN from accredited school or college.
• Valid RN or LVN license in good standing.
• Valid CPR and BLS card.

Responsibilities

• Coordinate RAI process including completion of MDS, CAA’s and development of a comprehensive care plan following RAI guidelines and facility policies.
• Ensure resident care plan is followed by interdisciplinary team and monitor progress for MDS compliance.
• Complete medical forms, charts, and reports accurately and timely.
• Assist DON/ADON or supervisors.
• Understand long-term care reimbursement including PDPM, Managed Care and Medicaid.
• Participate in scheduling resident care conferences.
• Participate in the QAPI process.
• Monitor Quality Care Indicators.
• Carry out quality improvement initiatives.
• Abide by facility policies including not disclosing user IDs and passwords.
• Report occupational exposures immediately.
• Participate in surveys and assist with corrective plans.
