Job summary

Advancednh is looking for a Minimum Data Set (MDS) Coordinator responsible for managing the Resident Assessment Instrument process. This role encompasses MDS assessments, care planning, and ensuring compliance with federal and state regulations.

The ideal candidate should have an active RN license, experience in long-term care, and strong knowledge of MDS 3.0 and CMS guidelines. The position requires excellent communication and organizational skills.

Qualifications

• Active RN license; LPN/LVN candidates may be considered.
• Minimum of 1-3 years experience in long-term care or skilled nursing.
• Strong knowledge of MDS 3.0, RAI process, and CMS regulations.

Responsibilities

• Coordinate MDS assessments and maintain compliance.
• Develop and update individualized care plans.
• Educate staff on documentation standards and regulatory requirements.
