Qualifications

• BA/BS degree required; minimum 2 years in social work or related experience.
• Experience in case management for older adults or individuals with disabilities preferred.

Responsibilities

• Lead Person Centered Planning process and document preferences, needs, and goals.
• Conduct assessments and develop Person Centered Support Plans (PCSP).
• Coordinate waiver programs (LTSS/IDD) and engage care teams for integrated plans.
• Mentor or precept new staff and participate in process improvement initiatives.
• Submit utilization/authorization requests with supporting documentation.
• Report critical incidents to state/county agencies as required.
• Assist with appeals, grievances, and state audits.
