Job summary

Elevance Health in Ohio is seeking an LTSS Service Coordinator-Clinician to support members with physical health, behavioral health, and long term services and supports. The role covers field visits to member homes and care sites, with some office meetings as needed.

Qualified candidates bring LPN/LVN, LSW, LCSW or LMSW licensure and at least 2 years' experience coordinating care for chronic illnesses or disabilities. A BA/BS in Health/Nursing is preferred.

Qualifications

• Requires licensure as LPN/LVN, LSW, LCSW, LMSW or RN-eligible in accordance with applicable state law and Nursing Diploma or AS in Nursing or a related field and minimum of 2 years of experience in working with individuals with chronic illnesses, co‑morbidities, and/or disabilities in a Service Coordinator or similar role.
• Current, unrestricted LPN/LVN, LSW, LCSW, LMSW or license other than RN (as allowed by state law) in applicable state(s) required.

Responsibilities

• Identify members at high risk and obtain clinical data.
• Identify members that would benefit from an alternative level of care or other waiver programs.
• Coordinate with RN to develop, monitor, evaluate, and revise the member's care plan to meet the member's needs.
• Coordinate care for members with chronic illnesses, co‑morbidities, and/or disabilities to ensure cost effective and efficient utilization of health benefits.
• Apply disease knowledge and clinical guidelines in decision making as directed by the RN.
