Qualifications

• Requires LPN/LVN, LSW, LCSW, or LMSW license (or RN alternative as allowed) and nursing or related degree.
• Minimum 2 years of experience with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator or similar role.
• Preferred: BA/BS in Health/Nursing; case management experience with older adults or individuals with disabilities.

Responsibilities

• Identify high-risk members and collect clinical data; coordinate with RN to develop and monitor care plans.
• Coordinate care for members with chronic illnesses or disabilities to ensure cost-effective use of health benefits.
• Participate in LTSS care coordination process within licensure scope and with health care teams.
