Qualifikationen

• Minimum 3 years in case management or related role.

• Perform telephonic and face-to-face functional assessments to identify and manage member needs.
• Manage members with chronic illnesses to ensure cost-effective use of benefits.
• Develop individual care plans with member, caregivers, and care team.
• Establish short and long-term goals and coordinate with providers and waivers as needed.
• Develop care plans and ensure member access to services across care continuum.
• Assist with authorizations/referrals within benefits structure or as permitted.
