Job summary

Elevance Health is seeking an LTSS Service Coordinator (Case Manager) to deliver person-centered care coordination in Ohio. You will manage a designated caseload within specialized programs, conduct assessments, and develop comprehensive care plans in collaboration with medical Directors and the member’s care team.

This field-based role emphasizes hands-on engagement, requires a BA/BS and 2+ years of social work experience, and offers a hybrid work approach with flexibility for travel to client

Qualifications

• BA/BS degree required with at least 2 years of social work experience.
• Experience with case management and coordinating services for long-term supports.
• Ability to conduct assessments and develop person-centered plans.

Responsibilities

• Perform face-to-face program assessments and identify care needs.
• Coordinate and manage a designated caseload across LTSS/IDD or behavioral health needs.
• Document goals and develop Person Centered Support Plans.
• Engage care teams and notify appropriate parties of changes or issues.
• Submit utilization requests and report critical incidents as required.
