Job summary

Elevance Health is seeking an LTSS Service Coordinator – RN Clinician (RN Case Manager) for Western Ohio. This field-based role travels to client sites and may include occasional office attendance.

The incumbent will manage member care plans across physical health, behavioral health, and long-term services, ensuring person-centered, coordinated care. The position requires an active Ohio RN license and at least 3 years in chronic illness-related case management, with strong communication and care

Qualifications

• Requires a high school diploma or GED equivalent and minimum of 3 years of experience in working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator, Case Management, or similar role.
• Current, active valid and unrestricted RN license in Ohio state(s) required.

Responsibilities

• Perform telephonic and face-to-face functional assessments to identify, evaluate, coordinate and manage member needs.
• Identify high-risk members and coordinate care with the health care team.
• Manage members with chronic illnesses to ensure cost-effective utilization of health benefits.
• Develop individualized care plans with short and long-term goals.
• Assist with authorizations/referrals within benefits structure or extra-contractual arrangements.
