Qualifications

Responsibilities

• Conduct face-to-face program assessments and coordinate waiver cases (LTSS/IDD, BH, PH).
• Identify members with potential clinical needs and act as single point of contact with the care team.
• Document member goals with the care team and assist in appeals, grievances, and audits.
