Qualifications

• BA/BS degree plus a minimum of 2 years of social work experience.
• Face-to-face program assessments using predefined questions.
• Ability to coordinate and manage LTSS/IDD waivers and BH/PH needs.
• Strong communication and advocacy for member needs.

Responsibilities

• Perform face-to-face program assessments and use motivational interviewing for evaluations.
• Identify members with potential clinical needs and coordinate with the care team.
• Manage non-clinical needs of members to ensure efficient use of long-term services.
• Document short- and long-term goals with the member and care network.
• Mentor or precept new staff and contribute to process improvements.
• Submit utilization/authorization requests with supporting documentation.
• Report critical incidents to state and county agencies.
• Assist with appeals, grievances, and state audits.
