Job summary

Qualifications

• Requires BA/BS degree and a minimum of 2 years of experience working with a social work agency.

Responsibilities

• Perform face-to-face program assessments using predefined questions.
• Coordinate member cases with clinical management and care teams.
• Document member goals and create Person Centered Support Plans (PCSP).
• Develop backup plans and coordinate waiver programs (LTSS/IDD).
• Submit utilization/authorization requests with supporting care plans.
• Report critical incidents to relevant agencies as required.
