Job summary

Elevance Health in Ohio is hiring a LTSS Service Coordinator to manage a designated caseload in specialized programs. You will lead Person Centered Planning, document preferences and goals, conduct assessments, and develop comprehensive PCSPs in collaboration with care teams.

Responsibilities include coordinating with Medical Directors, mentoring staff, submitting utilization requests, and ensuring compliant care across physical and behavioral health and long‑term supports.

Qualifications

• Requires BA/BS degree and a minimum of 2 years of experience working with a social work agency.
• Case management experience with older adults or individuals with disabilities preferred.
• BA/BS in Health/Nursing preferred.

Responsibilities

• Lead Person Centered Planning and document member preferences, needs, and goals.
• Create comprehensive PCSPs and backup plans with the care team.
• Coordinate with Medical Directors and participate in interdisciplinary rounds.
• Mentor or preceptor for new staff and contribute to process improvements.
• Submit utilization/authorization requests in line with the care plan.
• Report critical incidents to state and county agencies as required.
