• Perform face-to-face program assessments and coordinate LTSS/IDD, BH or PH needs.
• Identify members with potential clinical health care needs and coordinate with the clinical team.
• Document short and long-term service goals and care plans with the member and support circle.
• Submit utilization/authorization requests and report critical incidents as required.
