Job summary

Elevance Health in Wisconsin seeks an LTSS Service Coordinator (Case Manager) to conduct service coordination for a defined caseload in specialized programs. You will facilitate the Person Centered Planning process, document member goals, and work with the care team to deliver an integrated care plan.

The role requires field travel to client sites with occasional office meetings. A BA/BS and 2 years of social work experience are required; waiver experience and comfort with technology are

Qualifications

• Requires BA/BS degree and at least 2 years of social work experience.
• Experience with waiver programs (Family Care Partnership) preferred.
• Comfortable using technology.
• Travel to worksite and other locations as necessary.

Responsibilities

• Conduct face-to-face program assessments for identification and evaluation.
• Coordinate waiver needs and BH/PH guidance with the interdisciplinary team.
• Document short and long-term goals in the Person Centered Plan.
• Assist in training new staff and participate in process improvements.
• Submit utilization requests and report critical incidents per policy.
