Job summary

Elevance Health is seeking a LTSS Service Coordinator to manage a designated caseload and lead person-centered planning for members in LTSS/IDD programs. This field-based role travels to client sites with occasional office meetings.

You will collaborate with medical directors and interdisciplinary teams to develop integrated care plans, document goals, and engage support networks while adhering to state and federal regulations.

Qualifications

• Requires BA/BS degree and a minimum of 2 years of experience working with a social work agency; or any combination of education and experience which would provide an equivalent background.

Responsibilities

• Perform face-to-face program assessments using predefined questions to identify needs and coordinate a member's waiver (LTSS/IDD, BH or PH).
• Identify members with potential clinical health care needs and coordinate with the clinical management and interdisciplinary team to provide care coordination support.
• Manage non-clinical needs of members with chronic illnesses to ensure cost-effective utilization of long-term services and supports.
• Document member goals and plans in collaboration with their care team, including caregivers and physicians.
• Assist and participate in training of associates and process-improvement initiatives.
