Job summary

Elevance Health is seeking a LTSS Service Coordinator (Case Manager) to manage a designated caseload in specialized programs. You will collaborate with individuals to lead the Person Centered Planning process, documenting their preferences, needs, and goals.

Develop PCSPs and backup plans, while coordinating with Medical Directors to create an integrated care plan. Engage the individual’s support network, oversee physical and behavioral health management, and ensure adherence to state and

Qualifications

• Requires BA/BS degree and a minimum of 2 years of experience working with a social work agency; or any combination of education and experience which would provide an equivalent background.
• Preferred BA/BS in Health/Nursing or related field.

Responsibilities

• Lead Person Centered Planning processes, document preferences, needs, and goals.
• Conduct assessments and develop comprehensive Person Centered Support Plans (PCSP).
• Develop backup plans and coordinate with Medical Directors and interdisciplinary care rounds to establish integrated care plans.
• Engage the individual’s support network and oversee management of physical health, behavioral health, and long‑term services and supports.
• Mentor, subject matter expert, or preceptor for new staff and contribute to process improvements.
• Submit utilization/authorization requests with documentation supporting the care plan.
• Report critical incidents to appropriate internal and external parties (state and county agencies).
• Assist with appeals, grievances, and state audits.
