Job summary

Elevance Health seeks a LTSS Service Coordinator-Clinician to manage member care across physical health, behavioral health, and long‑term services and supports. This field-based clinician will perform assessments, coordinate with the care team, and develop care plans for members with chronic conditions and disabilities.

Qualifications

• Requires LPN/LVN, LSW, LCSW, LMSW or other RN-allowed license per state law.
• Minimum 2 years of experience serving individuals with chronic illnesses or disabilities in a service coordination role.
• BA/BS in Health/Nursing preferred.

Responsibilities

• Identify members with high risk of complications and obtain clinical data.
• Identify members who may benefit from alternative levels of care or waiver programs.
• Coordinate care with RN and health care team to ensure cost-effective use of benefits.
• Develop, monitor, and revise care plans to meet member needs.
• Apply clinical guidelines in assessments and care planning.
