Job summary

Elevance Health is seeking a LTSS Service Coordinator Clinician to support members in Elkhart, Kosciusko and Marshall counties, IN. The role operates primarily in the field with occasional office meetings, traveling to client sites and designated locations.

The candidate will work under an RN, performing licensure-bound activities to assist with assessments, care planning and care coordination for physical health, behavioral health and long-term services and supports.

Qualifications

• LPN/LVN or license other than RN per state law.
• Minimum of 2 years’ experience with chronic illnesses or disabilities in a service coordination role.
• Current, unrestricted LPN/LVN or social work license as allowed by state law.

Responsibilities

• Assist RN in identifying members for high risk complications.
• Obtain clinical data as directed by the RN.
• Identify members who may benefit from alternative levels of care or waiver programs.
• Provide collected information to the RN for verification and care planning.
• Coordinate care for members with chronic conditions with the health care team.
