Job summary

Elevance Health seeks a LTSS Service Coordinator (Case Manager) to manage a designated caseload across specialized programs. You will conduct assessments, lead Person Centered Planning, and document preferences and goals at the member level.

Qualifications

• BA/BS degree and a minimum of 2 years of experience in a social work setting.
• Experience with LTSS/IDD or care coordination is preferred.
• Strong communication and documentation skills to support person-centered planning.

Responsibilities

• Perform face-to-face program assessments and coordinate a member's waiver needs.
• Lead the Person Centered Planning process and document goals in PCSP.
• Develop backup plans and collaborate with medical directors and the care team.
• Engage the member's support network and oversee long-term services and supports.
