Job summary

Elevance Health seeks a LTSS Service Coordinator (Case Manager) to oversee a designated caseload in specialized programs, coordinating care across physical, behavioral health and long-term services. You will conduct assessments, document goals, and collaborate with a care team including family and physicians.

This field-based role travels to client sites with some office attendance for training. Elevance Health emphasizes trauma-informed, person-centered coordination in a hybrid work environment.

Qualifications

• Requires BA/BS degree and a minimum of 2 years of experience working with a social work agency.
• Experience in case management with older adults or individuals with disabilities preferred.
• BA/BS in Health/Nursing preferred.

Responsibilities

• Conduct assessments and develop comprehensive Person Centered Support Plans (PCSP) for designated caseloads.
• Collaborate with Medical Directors and participate in interdisciplinary care rounds to establish integrated care plans.
• Lead the Person Centered Planning process, documenting preferences, needs, and goals with the member and care team.
• Manage non-clinical needs of members to ensure efficient utilization of long-term services and supports.
• Submit utilization/authorization requests to utilization management and support state/federal compliance.
