Job summary

Elevance Health is seeking an LTSS Service Coordinator (Case Manager) to manage a designated caseload in LTSS/IDD, BH, or PH needs. The role blends field visits with occasional office attendance to coordinate comprehensive, person-centered plans.

You will collaborate with medical directors and a multidisciplinary team, document goals, and engage families and caregivers while ensuring compliance with state and federal regulations.

Qualifications

• Requires BA/BS degree and a minimum of 2 years of experience working with a social work agency.
• Preferred BA/BS in Health/Nursing.
• Willingness to travel for field-based work and engage with clients in their communities.

Responsibilities

• Manage service coordination for a designated caseload in LTSS/IDD, BH or PH needs.
• Lead Person Centered Planning, document preferences, needs and goals.
• Develop comprehensive Person Centered Support Plans and backup plans.
• Collaborate with Medical Directors and participate in interdisciplinary care rounds to integrate care plans.
• Engage the individual's support network and oversee management of health and long-term services.
• Submit utilization/authorization requests with proper documentation.
• Report critical incidents to state and county agencies and assist in audits.
