Job summary

Elevance Health is seeking an LTSS Service Coordinator (Case Manager) to support members across designated counties. This field-based role travels to client sites and occasionally attends meetings at the office, delivering hands-on coordination of long-term services and supports.

You will assess needs, develop Person Centered Support Plans, collaborate with Medical Directors and interdisciplinary care teams, and document goals with input from families and caregivers.

Qualifications

• Requires BA/BS degree and a minimum of 2 years of experience working with a social work agency or equivalent.
• Preferred BA/BS in Health/Nursing.
• Experience in case management for older adults or individuals with disabilities is preferred.

Responsibilities

• Manage service coordination for a designated caseload in LTSS programs.
• Conduct assessments and develop Person Centered Support Plans (PCSP).
• Work with Medical Directors and interdisciplinary care rounds to establish an integrated care plan.
• Engage the individual's support network and document goals with input from caregivers, family, and providers.
• May mentor or train new staff and participate in process improvement initiatives.
• Submit utilization/authorization requests with documentation aligned to the care plan.
• Report critical incidents to relevant state and county agencies and assist in appeals or audits.
