Job summary

Qualifications

• BA/BS degree required.
• Minimum of 2 years of experience with a social work agency or equivalent background.

Responsibilities

• Perform face-to-face program assessments for LTSS/IDD, BH or PH needs using predefined questions and motivational interviewing techniques.
• Identify members with potential clinical health care needs and coordinate with the clinical care management and interdisciplinary team.
• Manage non-clinical needs of members to ensure cost-effective and efficient utilization of long-term services and supports.
• Document short- and long-term service goals with the member's care team, including caregivers and providers.
• Identify members who could benefit from alternative service levels or other waiver programs; support appeals and audits as needed.
• May mentor or preceptor new staff and participate in process improvement initiatives.
