Qualifications

• BA/BS degree.
• Minimum of 2 years of experience working with a social work agency.
• Specific education, years, and type of experience may be required based upon state law and contract requirements.
• Travels to worksite and other locations as necessary.

Responsibilities

• Conduct face to face program assessments for identification and coordination of an individual’s waiver and health needs.
• Identify members with potential clinical health care needs and coordinate care with the clinical team.
• Manage non-clinical needs of members to ensure efficient utilization of LTSS.
• Document short and long-term service goals with the member and care team.
• Identify candidates who could benefit from alternative levels of service or other waiver programs.
• Mentor or precept new staff and participate in process improvements.
• Submit utilization/authorization requests with supporting care plan documentation.
• Report critical incidents to state and county agencies as required.
• Assist with appeals, grievances, and state audits.
