Job summary

Elevance Health is seeking a LTSS Service Coordinator in Indiana to manage a defined caseload, perform assessments, and develop Person Centered Support Plans in collaboration with the member and care team.

The role is field-based with travel to client sites, requiring a BA/BS and at least 2 years of social work experience. It supports hybrid work arrangements and mentorship for staff within a compliant, client-centered program.

Qualifications

• BA/BS degree. Minimum of 2 years of experience working with a social work agency; or any combination of education and experience which would provide an equivalent background.
• Specific education, years, and type of experience may be required based upon state law and contract requirements.
• Travels to worksite and other locations as necessary.

Responsibilities

• Conduct face-to-face program assessments using predefined questions and motivational interviewing techniques.
• Coordinate and manage an individual’s LTSS/IDD, BH or PH needs within applicable state law and contract.
• Develop and document Person Centered Support Plans (PCSP) and backup plans with the member and care team.
• Interface with Medical Directors and participate in interdisciplinary care rounds to support integrated care planning.
• Identify members who may benefit from alternative levels of service or other waiver programs, and document goals.
• Assist and participate in appeal processes, grievances, and state audits, and mentor new staff as needed.
