Elevance Health is seeking a LTSS Service Coordinator to support a Dallas County, IA caseload. You will perform assessments, coordinate LTSS/IDD and BH/PH needs, and develop a comprehensive Person Centered Support Plan with the member and care team.

You will travel to client sites, engage the member's supports, and document goals within integrated care plans. This role emphasizes collaboration and proactive care coordination.

Formación

• BA/BS degree required.
• Minimum 2 years of experience with a social work agency or equivalent background.
• Education/experience requirements may vary by state law and contract.

Responsabilidades

• Conduct face-to-face program assessments for identification and evaluation.
• Coordinate and manage an individual's waiver (LTSS/IDD), BH or PH needs.
• Develop a comprehensive Person Centered Support Plan and backup plan with the member.
• Interface with Medical Directors and participate in interdisciplinary care rounds.
• Engage the member's circle of support and manage LTSS needs within contract requirements.
• Document goals with the member and care team, including caregivers and physicians.
• Submit utilization/authorization requests with supporting care plans.
• Report critical incidents to state and county agencies as required.
• Assist with appeals, grievances, and state audits; mentor or train new staff as needed.
