Job summary

Elevance Health is seeking an LTSS Service Coordinator in Indiana to manage a defined caseload within specialized programs, conducting assessments and developing comprehensive Person Centered Support Plans (PCSP) in collaboration with the member and care team.

The role entails coordinating with Medical Directors, participating in interdisciplinary rounds, and overseeing PH/BH/LTSS needs while complying with state law and contract requirements. Travel to client sites is required.

Qualifications

• BA/BS degree required.
• Minimum of 2 years of experience with a social work agency or equivalent background.
• Education/experience requirements may vary by state law and contract.

Responsibilities

• Conduct face-to-face program assessments for LTSS/IDD and BH or PH needs.
• Coordinate and manage a member’s waiver and care with clinical teams to provide integrated care.
• Manage non-clinical needs to ensure cost-effective LTSS utilization.
• Document short- and long-term service goals with the member and care team.
• Identify members who could benefit from alternative services or other waiver programs.
• Mentor or train new staff and participate in process improvement initiatives.
• Submit utilization/authorization requests aligned with the care plan.
• Report critical incidents to state or county agencies.
• Assist with appeals, grievances, and state audits.
