Elevance Health in Wisconsin is hiring an LTSS Service Coordinator (Case Manager) to work in the field, traveling to member homes and facilities. You will conduct assessments, develop Person Centered Support Plans, and coordinate with an interdisciplinary team to meet PH/BH/LTSS needs.

As part of the Family Care program, you’ll engage with members, caregivers, and providers to maximize independent living and community participation, with hybrid work and periodic office visits as required.

Qualifikationen

• Requires BA/BS degree and a minimum of 2 years of experience working with a social work agency.
• Preferred field of study in health care related area.
• Waiver experience preferred.

Aufgaben

• Perform face-to-face program assessments using predefined questions and motivational interviewing.
• Identify members with potential clinical health care needs and coordinate with the care team as the single contact.
• Manage non-clinical needs to ensure efficient utilization of LTSS.
• Document goals in collaboration with the member's care team.
• May mentor new staff and participate in process improvement.
• Submit utilization/authorization requests with supporting documentation.
• Report critical incidents to state/county agencies.
• Assist with appeals, grievances, and audits.
