Qualifications

• Bachelor’s or higher degree required.
• Minimum 2 years of experience in a social work or related agency setting.
• Education/experience aligned with state and contract requirements.

Responsibilities

• Conducts face-to-face program assessments for waiver and LTSS needs.
• Identifies members with potential clinical needs and coordinates with the care team.
• Manages non-clinical needs to ensure cost-effective LTSS utilization.
• Documents goals with the member and care team per the plan.
• Identifies opportunities for alternative service levels or other waivers.
• Mentors new staff and supports process improvement initiatives.
• Submits utilization/authorization requests with care plan alignment.
• Reports critical incidents to state/county agencies as required.
• Participates in appeals, grievances, and state audits.
