Elevance Health in Dallas County, IA is seeking an LTSS Service Coordinator to manage a defined caseload, conduct assessments, and develop Person Centered Plans. This field-based role travels to client sites and supports individuals' health and LTSS needs in collaboration with care teams.

The position requires a BA/BS and at least 2 years of social work experience; travel is required, with some office days.

Competenze

• BA/BS degree. Minimum of 2 years of experience working with a social work agency; or any combination of education and experience which would provide an equivalent background.

Mansioni

• Conduct face-to-face program assessments for identification and evaluation using predefined questions.
• Develop and document Person Centered Plans (PCSP) and backup plans with the member and care team.
• Coordinate with clinical and interdisciplinary teams to support full integration of care plans and services.
