Job summary

Elevance Health is seeking a LTSS Service Coordinator Clinician to support the care coordination process under the supervision of a Registered Nurse. You will assist with field assessments and coordinate services to meet member needs across physical health, behavioral health, and long-term supports.

The role requires licensure as LPN/LVN or social work, at least 2 years in a related field, and willingness to travel. Hybrid/office engagement with training and meetings included.

Qualifications

• Field-based role requiring travel to client sites.
• Minimum 2 years of experience with chronic illnesses or disabilities in a care coordination setting.
• Current, unrestricted LPN/LVN or social work license as allowed by state law.

Responsibilities

• Assist RN in identifying members for high-risk situations.
• Obtain clinical data as directed by the RN.
• Identify candidates for alternative levels of care or waiver programs.
• Provide collected information to the RN for care plan development and monitoring.
• Coordinate care with the member and health care team to ensure cost-effective use of benefits.
