Elevance Health seeks a Field-based LTSS Service Coordinator to manage a defined caseload and coordinate person-centered planning for members. The role involves direct assessments, collaboration with care teams, and support across PH/BH/LTSS needs, with travel to client sites as required.

The position requires a BA/BS degree and at least 2 years of social work experience, with flexibility for alternate locations within commuting distance from a designated office.

Qualifications

• BA/BS degree required.
• 2+ years with a social work agency or equivalent background.
• Education/experience adjusted per state law and contract requirements.

Responsabilités

• Perform face-to-face program assessments for identification and coordination of LTSS/IDD, BH or PH needs.
• Coordinate member cases with clinical management and interdisciplinary team to provide care coordination.
• Manage non-clinical needs of members with chronic illnesses to ensure cost-effective utilization of LTSS.
• Document member goals with care team, including caregivers and providers.
• Identify members who may benefit from alternative service levels or other waiver programs.
• Mentor or train new staff and contribute to process improvements.
• Submit utilization/authorization requests with supporting care plan documentation.
• Report critical incidents to state and county agencies as required.
