Elevance Health is seeking an LTSS Service Coordinator to support a defined caseload in Indiana. The role involves field work, coordinating with clinical teams, and documenting person-centered plans with member goals and backup strategies.

The position requires a BA/BS and at least two years in social work. Applicants may travel to various sites and participate in care rounds to ensure integrated care for members while complying with state and contract requirements.

Qualifications

• BA/BS degree required.
• Minimum 2 years of experience with a social work agency.
• Experience with LTSS/IDD waiver processes is a plus.

Responsabilités

• Perform face-to-face program assessments using predefined questions.
• Identify members with potential clinical needs and coordinate with the care team.
• Document member goals and develop PCSP and backup plans.
• Submit utilization/authorization requests.
• Report critical incidents to state and county agencies.
• Mentor or preceptor for new staff as needed.
