Job summary

CVS Health is looking for a Case Management Coordinator in Illinois to support LTSS members with care coordination and education. The role requires 50-75% travel to meet members face-to-face and involves evaluating needs, coordinating care plans, and monitoring progress.

Ideal candidates have 2+ years in behavioral health or social/human services and case management, plus a valid IL driver’s license. A bachelor’s or master’s in behavioral health is preferred.

Qualifications

• Minimum 2 years of experience in behavioral health, social services or human services field.
• Minimum 2 years of case management experience.
• Must reside in Henderson, Hancock, Warren, or Knox County IL and be willing to travel 50-75%.
• Active Driver’s License preferred from IL.

Responsibilities

• Evaluate referred members’ needs and eligibility and plan case resolution.
• Collaborate with stakeholders to coordinate care plans and monitor progress.
• Escalate quality of care issues and provide coaching for member engagement.
• Support members in decision-making and navigating care options.
