Qualifications

• Requires a Bachelor's degree and 1 year of related experience or equivalent.
• RNs licensed in Illinois are preferred for Illinois Plan roles.
• LPNs with 1 year of relevant experience may be considered.

Responsibilities

• Evaluate member needs and available resources to plan best outcomes.
• Develop long-term care plans and identify providers and resources.
• Coordinate between members, families, and care teams to ensure access to services.
• Provide resource support for needs such as housing, employment, and independent living.
• Monitor care plans and outcomes and adjust as needed.
• Interact with long-term care providers and partners to meet needs.
• Collect and maintain member information to comply with regulations.
• May perform on-site visits to assess needs and collaborate with providers.
• Provide education on procedures, services, and benefits to members and families.
• Offer feedback to leadership to improve quality and cost-effectiveness.
