Centene Corp. is seeking a LTSS Service Care Coordinator to support community-based members with aging and physical disability waivers.

The role is hybrid-remote with 75% travel to member homes, focusing on developing and coordinating long-term care plans and educating families on available services and benefits. Ideal candidates have case management or advocacy experience, reside within 30-40 minutes of Des Plaines, IL, and hold a Bachelor's degree (or relevant licensure under Illinois plan)

Qualifications

• Requires a Bachelor's degree and 1 year of related experience.
• For Illinois Plan: RN license, or Bachelor/Master’s in Human Services, or LPN with 1 year of relevant experience.

Responsabilités

• Evaluate member needs and coordinate long-term care plans.
• Identify providers, specialists, and community resources for care.
• Coordinate between member/family and care team for accessible services.
• Educate members and families on procedures, options, and benefits.
• Document and maintain member information for regulatory compliance.
• May perform on-site home visits to assess needs.
