Qualifications

• RN licensed in Illinois OR BA/MA in Human Services OR LPN with 1 year elderly assessment experience.

Responsabilités

• Evaluates member needs and resources to coordinate best outcome plans.
• Develops long-term care or service plans and identifies providers and community resources.
• Coordinates between member/family and care provider team to ensure services are accessible.
