Job summary

Centene Corporation in Illinois is seeking a Care Manager to assist in developing and coordinating holistic care management for LTSS members. You will work remotely with 50% local travel for in-home visits and help educate members and families on services and benefits.

The role requires a Bachelor's degree and at least one year of related experience; RN licensed in Illinois or a related human services degree is preferred for the Illinois plan.

Qualifications

• Requires a Bachelor's degree and 1 year of related experience.
• RN licensed in Illinois is acceptable/preferred for Illinois Plan.
• Experience in case management, advocacy or home visits is preferred.

Responsibilities

• Evaluates member needs and resources to recommend/coordinate plans.
• Develops long-term care plans and identifies providers and resources.
• Coordinates between member/family and care providers to ensure services are accessible.
• Provides education to members and families on procedures, options, and benefits.
• Monitors plans, member status, and outcomes; provides recommendations.
• Performs on-site visits and liaises with providers.
