Centene is seeking a care management professional to assist in developing and coordinating holistic care plans for aging and disability waivers, with hybrid-remote duties and substantial home visit travel.

The role requires a bachelor’s degree with 1 year of related experience, or RN/LPN qualifications within Illinois. Training and ongoing education support are included, along with a competitive benefits package and a focus on quality outcomes.

Mansioni

• Evaluate member needs, resources, and coordinate the care plan for best outcomes.

• Coordinate between member/family and care teams to ensure services are accessible.
• Provide education to members and families on procedures, options, and benefits.
• Monitor plans and outcomes and update care plans accordingly.
