Job summary

Centene is seeking a Care Manager to support aging and physically disabled members by developing and coordinating holistic care plans and educating families on available services and benefits. The role requires collaboration with providers to ensure timely access to resources and compliance with regulations.

The position is hybrid-remote with 75% travel to member homes; preference for candidates living within 30–40 minutes of Des Plaines, Illinois.

Qualifications

• Bachelor's degree and 1 year related experience.
• RN license in Illinois preferred.
• Willingness to travel and residency near Des Plaines area.

Responsibilities

• Evaluates member needs, resources, and facilitates optimal care plans.
• Assists in developing long-term care plans and coordinates with providers.
• Coordinates between member/family and care teams to ensure access to services.
• Provides resource support for employment, housing, and independence needs.
• Monitors plans and member outcomes; recommends adjustments as needed.
• Interacts with long-term care providers to meet member needs.
• Collects and maintains member information in compliance with regulations.
• May perform on-site visits to assess needs and collaborate with providers.
• Educates members and families about procedures, options, and benefits.
