Job summary

Centene seeks a Care Manager to assist in developing, assessing, and coordinating holistic LTSS care for Illinois members. You will educate members and families on services and benefits and help create personalized service plans for long-term care needs.

The role blends remote work with roughly 50% local travel for in-home visits, serving members with Physical Disability and Aging waivers. Training Monday–Friday CT; strong preferences for case management and advocacy experience.

Qualifications

• Requires a Bachelor's degree and 1 year of related experience.
• RN licensed in Illinois is preferred.
• Bachelor or Master’s Degree prepared in human services related field or LPN with relevant experience.

Responsibilities

• Evaluate member needs and resources to recommend and/or facilitate the care plan.
• Develop and coordinate long-term care plans and identify providers and resources.
• Coordinate between member/family and care provider team to ensure services are accessible.
• Provide education on procedures, healthcare options, and benefits to members and families.
• Monitor care plans and outcomes and adjust as needed.
• Ensure compliance with policies and standards.
